What the Toolkit is

The Toolkit provides guidance on improving palliative care for people with intellectual disability and has been developed to support the implementation of the Tailored model of palliative care for people with intellectual disability. By utilising the Toolkit in conjunction with the Tailored model of care, health professionals have the opportunity to transform the quality of palliative care for people with intellectual disability.

Who should use the Toolkit

The Toolkit is primarily designed for health professionals and service managers within generalist and specialist health services providing palliative care.

We recommend you view the Tailored model of care document first for context before using the Toolkit.
